Drugs and other forms of medicine have been referenced frequently throughout the Homestar Runner universe, in both medicinal and recreational contexts.

Medicinal Drugs

• Email funny — Strong Sad asks Strong Bad, "Did you take some of my pills again?"
• Peasant's Quest — One of the items you give The Three Keepers of Trogdor is a bottle of pills.
• Email time capsule — One of the Easter eggs features Jobar's Big Ol' Headache Medicine.
• Stinkoman 20X6 — The main item for restoring health is a large pill.
• Email candy product — Strong Bad's junkmail filter, Edgar's Baby's Daddy, has one email with the subject "Re: PharamBlacy news", parodying spammers who advertise prescription drugs.
• Email alternate universe — Homestar Runner tries to make a real fruit smoothie using "Total Load", presumably vitamin supplement pills.
• No Hands On Deck! — Strong Sad states that he "needs to get his prescription changed".
• Email retirement — Strong Bad runs out of his "antidepressants", and later his "Cloud Shape Spotting Log" in an Easter egg lists "diet pills".
• Email email thunder — Homestar claims that he accidentally took some of Strong Sad's pills before the events in long pants.

Recreational Drugs

• Scroll Button Songs — Coach Z raps, "And if you're tryin' to fade me then ya must smoke crack!"
• Limozeen Thanksgiving E-Cards — Gary Palaroncini says, "Kids under six, don't drink and drive! And drugs, stay out of school!".
• Email autobiography — In "Words I Probably Said", Strong Bad explains that "somebody must have slipped [Reg] some drugs or something before the game".
• Email 4 branches — Homestar's "deadbeat boyfriend" shirt bears a striking similarity to a "Legalize Marijuana" T-shirt.
• Teen Girl Squad Issue 13 — So and So confuses diet soda with PCP.
  • In the DVD Commentary, Ryan Sterritt mentions a pot leaf visible in the Nuthin' but a "G" Thang content warning music video.
• Character Peacey P — The rapper's name is derived from PCP.
  • Rap Song — the Toon Menu channel ID for this short is PCP.
• Homestar Ruiner — Strong Bad orders a free sample of Total Load Strength Enhancer(steroids).
