Drugs and other forms of medicine have been referenced frequently throughout the Homestar Runner universe, in both medicinal and recreational contexts.

- The theme from DANGERESQUE II: This time, it's not Dangeresque I — Dangeresque claims to be a celebrity pharmacist.
- Peasant's Quest — One of the items that is given to The Three Keepers of Trogdor is a bottle of pills.
- Email time capsule — One of the Easter eggs features Jobar's Big Ol' Headache Medicine.
- Stinkoman 20X6 — The main item for restoring health is a large pill.
- Email candy product — Strong Bad's junkmail filter, Edgar's Baby's Daddy, has one email with the subject "Re: PharamBlacy news", parodying spammers who advertise prescription drugs.
- Email web comics — The Cheat's junk mail filter has an email with the subject "PharmC7acy".
- Email alternate universe — Homestar Runner tries to make a real fruit smoothie using "Total Load", the total body fitness enhancing powder.
- Email retirement — Strong Bad runs out of his "antidepressants", and later his "Cloud Shape Spotting Log" in an Easter egg lists "diet pills".
- Fan Costumes '08 — In mocking a The King of Town costume, Strong Bad uses the phrase, "I don't think the medication is working."
- Email sbemail 206 — An animatronic Old-Timey Strong Bad advertises Olde Time Jones Drug Filled Tonic.
- Email the next april fools thing — Strong Bad is inspired to think of File Folder-Shaped Vitamins.
- Decemberweenvent Calendar (Day 17) — Walking through his living room at night, Homestar Runner sees his cow lamp morph into a semi-realistic, talking character and concludes that he took too much cough syrup.

- Scroll Button Songs — Coach Z raps, "And if you're tryin' to fade me then ya must smoke crack!"
- Limozeen Thanksgiving E-Cards — Gary Palaroncini says, "Kids under six, don't drink and drive! And drugs, stay out of school!"
- Email autobiography — In "Words I Probably Said", Strong Bad explains that "somebody must have slipped [Reg] some drugs or something before the game".
- Commandos in the Classroom — Blue Laser Commander asks Reynold if he wants to "smoke some candy" in a PSA-like video.
- Email 4 branches — Homestar's "deadbeat boyfriend" shirt bears a striking similarity to a "Legalize Marijuana" T-shirt.
- Email strong badathlon — When he loses the Remote Put, Strong Bad accuse the "East German" of taking steroids, demanding the judges check his pants.
- Teen Girl Squad Issue 13 — So and So confuses diet soda with PCP.
- In the DVD commentary, Ryan Sterritt mentions a pot leaf visible in the Nuthin' but a "G" Thang content warning music video.
- Peacey P — The rapper's name is derived from PCP.
- Rap Song — the TV Time Toons Menu's channel ID for this short is PCP.
- Homestar Ruiner — Strong Bad orders a free sample of Total Load Strength Enhancer, a type of steroid.
- Email sbemail 206 — After Old-Timey Strong Bad advertises Olde Time Jones Drug Filled Tonic, he asserts that "It's for children, you!".
- Characters from Yonder Website — After Strong Sad sees Strong Bad and others passed out and babbling, he finds that they were drinking Smarty Juice which is known to cause "drowsiness, euphoria, and unbelievably soothing children's programming." He then joins in by drinking some.
- @StrongBadActual — Strong Bad claims to not remember the events of Poker Night at the Inventory due to taking a bunch of dry Total Load powder at once (or, as he calls it, "chalkmouthing") before waking up the next day on a pile of unsold SBCG4AP DVDs.
