Drugs and other forms of medicine have been referenced frequently throughout the Homestar Runner universe, in both medicinal and recreational contexts.

- The theme from DANGERESQUE II: This time, it's not Dangeresque I — Dangeresque claims to be a celebrity pharmacist.
- Peasant's Quest — One of the items that is given to The Three Keepers of Trogdor is a bottle of pills.
- Email time capsule — One of the Easter eggs features Jobar's Big Ol' Headache Medicine.
- Stinkoman 20X6 — The main item for restoring health is a large pill.
- Email candy product — Strong Bad's junkmail filter, Edgar's Baby's Daddy, has one email with the subject "Re: PharamBlacy news", parodying spammers who advertise prescription drugs.
- Email web comics — The Cheat's junk mail filter has an email with the subject "PharmC7acy".

- Characters from Yonder Website — After Strong Sad sees Strong Bad and others passed out and babbling, he finds that they were drinking Smarty Juice which is known to cause "drowsiness, euphoria, and unbelievably soothing children's programming." He then joins in by drinking some.
- @StrongBadActual — Strong Bad claims to not remember the events of Poker Night at the Inventory due to taking a bunch of dry Total Load powder at once (or, as he calls it, "chalkmouthing") before waking up the next day on a pile of unsold SBCG4AP DVDs.
